Calc • Format Reports from Calc

I need to create a formatted report from my OOCalc rows. The report needs to be in OOWriter or some other text app. Here's my spreadsheet (it's all straight text with column titles in row 1):

Name          Address          City          State  PhoneJohn Smith    100 Main St      Springfield   IL     328-874-0868Jane Brown    13 Wash Blvd     Bluff         OK     601-591-3422

Here's what the report needs to look like:
John Smith
100 Main St
Springfield IL
328-874-0868

Jane Brown
13 Wash Blvd
Bluff OK
601-591-3422

Does anyone have a straight forward way to do this?
